DOCUMENTS REQUIRED
Identity Documents
- Provide notarized copies of two identification forms, including one passport, to verify your current name without abbreviations. Submit additional identity documents for any past names.
- Acceptable forms of photo identification (ID) are:
- Passports (required)
- Driver's License(s)
- Other government-issued photo identification Other acceptable forms of identification include:
- Birth Certificates
- Marriage Certificates
- Divorce Certificates
- Legal Name Change Affidavits
- Photo IDs must feature a clear, current (non-expired) colour photo.
- You are responsible for fees. Non-English documents need certified translation.
- CGFNS accepts photocopies, not originals.
Employment History
- NCNZ's employment standards details are on their website.
- CGFNS needs a self-declaration to prove two years (2500 hours) of nursing work in the last five years. You can still apply to CVS-NCNZ for credential assessment if you're working on meeting this requirement.
Education History
- Provide evidence of your education qualifications for nursing practice leading to licensing/registration.
- CGFNS will communicate how to submit this information via email after your initial application.
- For direct submission by your education institution:
- Send a nursing education form (downloadable from the Applicant Portal).
- Include a complete academic transcript, detailing courses and credit/clock hours.
- Include supplemental documentation based on your country/jurisdiction:
- Related Learning Experience (Philippines).
- School and university mark sheets (India).
- Diploma supplement (European Union countries).
- Evidence of your clinical experience hours (United States).
- If directed to submit education information yourself:
- Complete a legal self-declaration to CGFNS about your nursing education.
- Upload copies of your academic transcript and qualification certificate.
- Request these documents from your academic institution when advised by CGFNS.
Language Proficiency
- Demonstrate English language abilities in reading, writing, listening, and speaking through approved language proficiency test results, unless you qualify for an English Language Evidence Pathway (see below).
- Approved Testing Vendors and Required Scores:
- Occupational English Test (OET)
- Prior to August 2018: Minimum B in reading, writing, listening, and speaking.
- After September 2018: Minimum 350 in reading, listening, and speaking, and 300 in writing.
- International English Language Testing System (IELTS) - Academic Module
- Minimum 7 in Reading, Listening, and Speaking, and 6.5 in Writing.
- You can achieve the required scores over multiple test sittings but within 12 months of the first test and three years of CGFNS report transmission to the Nursing Council.
- English Language Evidence Pathways may apply if your nursing education was conducted in English in certain countries (UK, Singapore, Ireland, Canada, or the USA) or if your registration in these countries required English language testing.
- CGFNS will submit alternate pathway requests to NCNZ with the CVS-NCNZ program report.
- Note: Failure to provide English Proficiency Testing evidence within a year and ineligibility for alternate pathways will result in order expiration.
